VIDEO RELEASE FORM FOR MINORS UNDER 18.

I, the undersigned parent/legal guardian of minor(s) must list all minors in form below, hereby grant permission for my child to be photographed and/or filmed during events organized by Science Hill Church

I understand that the photographs and/or videos taken during the event may be used for promotional purposes, including but not limited to social media, websites, and other promotional materials.

I hereby release Science Hill Church from all claims, liabilities, and demands arising out of or in connection with the use of said photographs and/or videos, including but not limited to any claims for defamation, invasion of privacy, or infringement of moral rights.

I understand that my child's full name may be used in conjunction with the photographs and/or videos. If I wish to revoke this permission, I will notify Science Hill Church in writing.

I acknowledge that Science Hill Church is not responsible for any misuse of the photographs and/or videos by third parties once they are in the public domain.
